Technical Note:
PC Users
Our online form uses Adobe Acrobat Reader 5 or greater plug-in to conveniently submit the form from home or work. It is important that you have at least version 5 of the plug-in to successfully use our online form.
Mac Users

Case Study 15 Cosmetic Periodontal Surgery III: Severe Anterior Wear with Supra-Eruption of Lower Incisors
In this case, severe attrition caused a supra-eruption of the lower incisors. This eruption/wear process brought the gingiva and alveolar bone with it coronally causing complete esthetic disharmony. In centric occlusion, only the gingiva was visible around the lower incisors. We used a surgical crown lengthening procedure to correct this. After six weeks of healing, the teeth were crowned to complete the esthetic transformation (restorations by Dr. Greg Linney).
