PC Users
Our online form uses Adobe Acrobat Reader 5 or greater plug-in to conveniently submit the form from home or work. Please download the free plug-in from Adobe's web site if it is not already installed on your system. It is important that you have at least version 5 of the plug-in to successfully use our online form.
Mac Users
You must open and submit the form in a Safari Browser with the latest Mac operating system. It is also important to have the latest version of Adobe Acrobat Reader on your computer in order to submit your form to our office correctly, please download the free plug-in from Adobe's web site.
In many situations, a tooth is fractured, unrestorable, or is failing due to advanced periodontal disease (i.e. deep furcations, etc.). A bridge may not be an option due to lack of posterior abutments, therefore, a removable appliance (RPD) or an implant may be the only choice. If implants are the treatment of choice, traditionally, the tooth is extracted and a transitional period starts with the healing of the extraction socket and, eventually, placement of the implants. This transitional period is not a good selling point to patients concerning implants. If they have to wait 6-9 months for a suitable restoration, they may decide not to pursue this treatment.
One option we have now is the immediate implant. The success rates are comparable to traditional placement (over 95%). In our practice, we have not had any early failures with immediate implants. The benefits of this are a reduced waiting period for the final restoration and only having one surgery. With ITI™ (Straumann) implants there is no second stage surgery either, so there is only 1 step. Three months later, the implants are ready to be restored.
In this case, the patient is a 58-year-old female who presented to our clinic with pain in the lower right quadrant and suppuration coming from #31 sulcus. A radiolucency exists surrounding the mesial root. This tooth was endodontically treated over 10 years ago, with no complications. A vertical root fracture was suspected and confirmed with an exploratory procedure. In this situation, the bridge was sectioned and #31 extracted. Immediately, 2 implants were placed in the #30 and 31 position. A bone graft and guided bone regeneration membrane (Calcitek™ BioMend) was placed around the #31 implant and the soft tissue was sutured around the implants. These implants will be restored in 3 months.
Healing is usually uneventful in these cases. Precaution must be taken to avoid the mandibular nerve and vessels. Stability of the implant must be achieved. In most cases of single rooted teeth and mandibular molars, immediate implants can be accomplished. Maxillary molars may need a traditional healing period due to the proximity of the 3 roots.
