In the last 2 years, implants have been available that replace molars more realistically. Straumann ITI wide neck-wide body implants have a fixture width of 4.8 mm and a neck width of 6.5 mm. This allows for a smooth transition between the implant fixture and crown. This also accomodates for a larger crown that looks more realistically like a molar. The one-stage design also eliminates the implant/abutment interface at the crest of the alveloar bone (or even below in the case of countersinking) which can create chronic inflammation and bone loss.
I frequently see either small premolar size crowns placed on molar position implants, or a molar size crown on a small implant (with an abrupt transition from implant fixture to crown). The latter situation does not allow for adequate cleaning. Chronic inflammation usually persists. Implants must be cleansible, like teeth.
